flash email using php Options

Any chance I could get some help perhaps here related to the below code?
This seems to be a comune code for flash email using php.

The html code contains

the php code

the flash code is pretty simple and other then gathering subject, username and comments, I don't have
anything fancy.

Does the php needs to be compiled somehow?
